Staying HomeCare in California seeks a compassionate Care Manager for a part-time, on-site role in El Segundo. You will be the primary contact for clients and families, supervise field caregivers, and coordinate with the Office Manager on scheduling and care quality.

Ideal candidates have 3+ years in home care supervision, strong client assessment and care-planning skills, excellent communication, and a valid California driver’s license with reliable vehicle.

Formación

• Minimum 3 years of experience in a home care, home health, or senior care setting in a supervisory, coordinator, or care management role
• Demonstrated ability to conduct client assessments and develop individualized care plans
• Strong interpersonal skills with experience communicating directly with clients, families, and caregivers in emotionally sensitive situations
• Ability to work independently in the field with reliable transportation
• Familiarity with California Home Care Organization (HCO) regulations and compliance standards
• Proficiency with care management or home care software for documentation and care plan management
• Valid California driver's license and reliable personal vehicle
• Experience with post-surgical recovery care, dementia care, or disability support services

Responsabilidades

• Conduct in-home assessments and develop individualized care plans
• Match clients with caregivers based on skills and schedule
• Oversee caregiver field supervision and provide real-time coaching
• Maintain HIPAA-compliant client documentation and care plans
• Coordinate with Office Manager to ensure scheduling reflects current care plans
• Ensure CA HCO standards and agency policies are followed
