Job summary

The Brook Hospital - KMI in Louisville, KY is seeking a part-time Referral Coordinator to handle incoming intake calls, assess appropriate level of care, and complete admission paperwork.

You'll collaborate with hospital units, medical staff, and referrals sources to coordinate Extended Care and Transitional Living referrals. This role supports a full continuum of services for children to adults and offers a loan forgiveness program and comprehensive benefits.

Qualifications

• Master's Degree in Behavioral Health from an accredited school.
• 0-1 years of related experience; preferred: 1-3 years.

Responsibilities

• The Referral Coordinator is responsible for answering all incoming intake calls and providing appropriate resolution.
• Conducts initial evaluations to determine an appropriate treatment level of care.
• Responsible for completing admission paperwork, including admission data entry.
• Works with all hospital units, business office, medical staff, medical records, utilization review, marketing, nursing, and data processing.
• Receives and coordinates Extended Care, Star, and Transitional Living House referrals through communication with providers and department managers.
