Job summary

Qualifications

• MSW required and state licensure as applicable.
• 0–2 years related experience preferred.
• Masters in Social Work with knowledge of dialysis patient needs.

Responsibilities

• Assess patients' psychosocial status and needs to support rehabilitation and QoL.
• Collaborate with the healthcare team to identify effective interventions.
• Provide counseling and refer to community resources for ongoing support.
• Educate staff on CKD psychosocial impact and patient education programs.
• Document MSW interactions and adhere to policy requirements.
