Job summary

Responsibilities

• Assess patients' psychosocial status, strengths and needs as part of comprehensive patient assessment.
• Participate in care planning with the patient and healthcare team to identify effective interventions that meet rehabilitation, treatment goals and improve quality of life.
• Utilize Fresenius Kidney Care education programs and social work theory to address barriers and meet patient treatment goals.
• Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
• Provide supportive counseling services within the scope of clinical training and state license.
• Provide educational counseling to patients seeking transplant.
• Refer patients to community resources (home health services, vocational rehabilitation) to enhance outcomes.
• Maintain knowledge of local vocational/educational rehab programs and assist with referrals.
• Assess patient awareness of advance directives and facilitate access to directives when needed.
• Discuss DNR status with physician/nurse to ensure informed decisions.
• Adhere to FMCNA Social Work Policy and documentation requirements.
• Document MSW interactions and interventions for patients and families.
