Job summary

Fresenius Medical Care is seeking a Social Worker to provide psychosocial services to in-center and home dialysis patients. You will assess psychosocial status, participate in care planning, and offer counseling to support treatment goals and quality of life.

In collaboration with the healthcare team, you will educate patients and families about transplantation options, and connect them with community resources to facilitate adherence and recovery. A state license and MSW are required.

Qualifications

• 2–5 years of related experience.
• MSW degree required.
• State license required.

Responsibilities

• Assess patients' psychosocial status and plan care in collaboration with the interdisciplinary team.
• Provide counseling and educational support to patients and families.
• Support psychosocial needs to improve adherence and quality of life.
• Assist with transplant education and referral processes.
