Job summary

Live Well Homecare is seeking an registered nurse to provide direct patient care including assessment, planning, implementation and evaluation. You will educate patients and families, develop personalized care plans, and coordinate with the healthcare team to meet physical, psychosocial, and emotional needs while ensuring safety and regulatory compliance.

Preferred candidates have a Bachelor's Degree with at least 2 years of experience.

Role Overview:

Provides direct patient care including assessment, diagnosis, planning, implementation, and evaluation within nursing standards. Promotes patient and family-centered care, educates on illness management and post-treatment needs, and ensures safety, quality, and regulatory compliance. Develops and documents personalized care plans in collaboration with the healthcare team and patients, addressing physical, psychosocial, and emotional needs while respecting individual values and barriers. Uses critical nursing skills to evaluate patient conditions and communicate care plans effectively.

Qualifications:

Requires a Nursing Diploma or Associate's Degree, RN license, and may benefit from a Bachelor's Degree with at least 2 years of experience. No supervisory experience needed. Preferred qualifications include emergency medicine RN experience.
