Glacier Hills Senior Living Community in Ann Arbor, MI is seeking a compassionate Part-time Day Shift Discharge Planner (Social Worker) to support residents and families through transitions within our Skilled Nursing community.

You will conduct resident assessments, coordinate care, connect families with community resources, and document progress as part of an interdisciplinary team.

Qualifications

• Licensed Social Worker (LSW) in Michigan.
• Bachelor's or Master's degree in Social Work or related human services field.
• Healthcare social work experience preferred.
• Experience with older adults or in long-term care preferred.
• Compassionate, resident-centered approach with strong communication skills.

Responsabilités

• Assist with resident assessments, care planning, and discharge planning.
• Conduct cognitive and mood evaluations and coordinate follow-up services.
• Provide emotional support, guidance, and education to residents and families.
• Connect residents with community resources, financial assistance programs, and social service agencies.
• Maintain timely documentation and collaborate with the care team to support resident well-being.
