Job summary

PIONEER HEALTHCARE MANAGEMENT in Northville, MI is seeking a Part-Time Social Worker to identify residents' social, emotional and psychological needs and develop plans to support their stay.

You will coordinate with the interdisciplinary team, conduct assessments within 3 days of admission, manage care conferences, discharge planning and education for residents and families about post-acute options.

Qualifications

• Bachelor's degree in social work or related human services field required.
• 1 year supervised social work experience in a health care setting required.

Responsibilities

• Complete and document Social Work and Psychosocial assessments for all residents within 3 days of admission or readmission.
• Ensure 3877/78 forms are completed and accurate, upon admission and annually.
• Schedule and document 72-hour Resident Care Conference for new admissions and readmissions; notify IDT by email.
• Complete MDS sections and the appropriate CAAs.
• Update Care Plans including Psychosocial, Discharge Planning, Behavioral, Cognitive loss, mood, vision, hearing, Podiatry and dental; ensure diagnoses and medications are on record.
• Coordinate care with psych provider including medication management and consents.
• Coordinate ancillary services (Dental, Ophthalmology, Audiology) with input of DON/Administrator.
• Educate residents and families regarding Advance Directives; complete forms within 48 hours of admission.
• Attend weekly Medicare meetings and monthly QAPI meetings with prepared data.
• Demonstrate proficiency with EMR and maintain frequent contact with residents and families.
• Involve in in-service training of aides and staff related to resident rights and Social Services.
• Assist with discharge planning and communication of resident rights related to discharge.
