Prairie Community Services seeks a Direct Support Provider to assist individuals at Havenwood Home in Wheaton, MN. This part-time, on-site position supports daily living activities, personal cares, and documented daily charting.

The role requires a DHS background check, a valid driver’s license, and ability to read, write and communicate in English. Starting wage is $16.50–$17.50 per hour, with competitive benefits.

Qualifications

• Must be 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.

Responsibilities

• Provide training and assistance with daily personal cares, medication administration, and daily charting.
• Support community activities, money management, meal preparation, laundry and cleaning.
