Prairie Community Services, Inc. is seeking a Direct Support Provider for Summerwood Home in Mankato, MN. The role provides daily personal care, medication administration, and community activities under person-centered plans.

Starting wage is $16.00–$18.00 per hour. This on-site, full-time position requires a valid driver’s license and DHS background clearance, with opportunities for comprehensive benefits and flexible scheduling.

Qualifications

• Must be 17 years old or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must read, write, communicate and understand the English language while conducting company business.

Responsibilities

• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.
