Job summary

Prairie Community Services in Minnesota is seeking a Direct Support Provider to supervise and care for individuals in daily living, recreation, and community activities. You will assist with daily living tasks, meal prep, laundry, and safety while promoting independence and person-centered planning.

The role starts with a wage of $17.00-$19.00 per hour and requires English fluency, a valid driver’s license, and the ability to meet DHS background checks.

Qualifications

• Minimum age 17 or as licensure dictates.
• Pass DHS background clearance.
• Valid driver’s license.
• Able to read, write, communicate and understand English in business context.
• Read, comprehend and follow each person served’s program plans and procedures.
• Deliver support based on person-centered services.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Assist with daily living activities, recreation, community orientation.
• Support with medication management, meal prep, laundry and cleaning.
• Maintain documentation of daily program information.
• Respond to needs with CPR/First Aid when required.
