Responsibilities

• Assist individuals receiving services in leading self-directed lives and to participate fully in their local communities.
• Support emotional, physical, and personal well-being while monitoring and reducing risk of harm.
• Promote human and civil rights and help individuals understand their rights and responsibilities.
• Explain information and teach individuals in age- and developmentally appropriate communications.
• Maintain confidentiality per state and agency regulations, breaking it only if there is imminent harm.
• Follow agency policies and OPWDD regulations in all services and supports.
• Identify and report neglect or abuse and participate in investigations as required.
• Implement each individual's Hab plan and document data in the EMR system.
• Provide a therapeutic and professional environment and maintain professional behavior.
• Communicate with family, service coordinators, and other life contacts as directed.
