Oak Hill in Lebanon, CT is seeking a Direct Support Professional (Part Time, Day Shift) to support individuals with intellectual and developmental disabilities in a residential setting.

You will assist with daily living, health monitoring, medication administration, meals, mobility, and community participation while following health care plans and regulations. Paid training, benefits, and career development are offered.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment.
• Commitment to continuity of care, including occasional extended shifts.

Responsibilities

• Support individuals in living healthy, meaningful, and fulfilling lives.
• Assist with personal care, grooming, hygiene, and daily living activities.
• Provide continence care as needed and encourage independence.
• Monitor health information and document vitals as required.
• Administer medications and maintain medication records per training and directives.
• Transport individuals to appointments and activities using agency vehicles.
• Prepare meals and snacks per menus and dietary needs.
• Assist with mobility and transfers using approved equipment.
• Document daily activities and health information in the EHR.
• Follow safety procedures and emergency protocols; apply behavioral support strategies.
• Communicate effectively with team, individuals served, families, and stakeholders.
