AHRC Nassau is seeking a Community Habilitation Professional to provide support to individuals with developmental disabilities in their homes or community settings. This part-time role involves assisting with daily living activities, ensuring safety, and documenting progress. Ideal candidates will have a high school diploma and experience in the field, along with strong communication and organizational skills.

Qualifications

• Prior experience supporting individuals with intellectual and developmental disabilities preferred.
• Valid NYS driver’s license compliant with agency’s insurance standards.
• Comfortable transporting individuals in your personal vehicle.

Responsabilités

• Support and assist with activities of daily living including personal hygiene tasks.
• Provide companionship, supervision, and instruction ensuring safety.
• Document service implementation and individual progress in the Medisked system.
