Job summary

Open Cities Health Center in St. Paul, MN seeks a Community Outreach Service Coordinator to design, implement, and evaluate outreach programs that promote patient access and health equity, and support fundraising efforts.

Reporting to the COO, the CSC leads community health initiatives, develops grant proposals, directs marketing materials, and oversees the MNSure Navigator program while coordinating with partners.

Qualifications

• Bachelor’s degree in business, public or health administration or closely related field.
• Experience with underserved community health programs.
• Experience developing and operationalizing growth strategies.

Responsibilities

• Manages community health programs from planning to evaluation.
• Identifies grant opportunities and assists with proposals.
• Leads development of marketing materials and program promotion.
• Oversees MNSure Navigator program and coordinates with leaders to ensure functionality.
• Advances health equity efforts within department and community partnerships.
• Represents OCHC in meetings with public agencies and associations.
• Disseminates information on health promotion and equity to leadership.
• Supervises staff, conducts performance evaluations, and addresses issues.
• Maintains participant records and program reports.
