Responsibilities

Identify, develop, and maintain collaborative partnerships with community-based organizations.
Partner with community-based organizations to develop health and wellness initiatives to improve community health.
Act as point of contact for community organizations and stakeholders and provide information on engagement efforts.
Assist patients and staff in identifying appropriate community resources to improve access to care and services.
Prepare and present educational materials and presentations for community events and initiatives.
Advocate for policy initiatives affecting community health and wellbeing.
