Responsibilities

• Lead the Community Health Needs Assessment (CHNA) and Implementation Plan/Strategy processes.
• Coordinate community benefits data collection, tracking and regulatory reporting.
• Develop and support community programs, events and partnerships aligned with identified health needs.
• Serve as liaison to nonprofits, public health agencies, and municipal partners.
