Mass General Brigham Community Physicians, Inc. seeks a Community Health Outreach Worker to engage diverse communities, provide education, support, and help navigate the healthcare system.

You will conduct outreach, deliver health coaching, and assist with referrals and screenings under supervision. This full-time role offers a hybrid schedule with 3–4 days in office or community sites in Hyde Park, Dorchester, West Roxbury, and nearby areas, plus 1–2 remote days.

Competenze

• High School Diploma or equivalent required.
• Associate's or Bachelor's Degree preferred.
• Experience in community outreach or health education preferred.

Mansioni

• Conduct community outreach and engage community members to understand their health needs.
• Provide health education and coaching to individuals and groups.
• Assist community members in accessing and navigating the healthcare system, including referrals.
• Provide basic health screenings and assessments to identify health risks.
• Maintain records of community health outreach activities and interventions.
