Job summary

CONCERN seeks a part-time Community Based Caseworker in Wilkes-Barre, PA to support children and families through casework counseling, visits, and reunification services. You will collaborate with county agencies, maintain client records, and ensure HIPAA compliance while coordinating transportation and resources.

The role demands strong organizational, written, and interpersonal skills, a valid driver’s license, and the ability to work varied hours including nights and weekends.

Qualifications

• High School diploma with five years of experience in the child services field working with children and families (Required)
• Bachelor's degree in Human Services or related field and two years of related experience and/or training (Preferred)
• You will maintain all required clearances.
• You will maintain a valid driver's license and have reliable transportation.
• You will demonstrate excellent written, verbal, organizational, and interpersonal skills.
• You will abide by HIPAA regulations.

Responsibilities

• Meet regularly with clients and families as authorized by the referring county agency to prevent out-of-home placements or aid in reunification through casework counseling, visit coaching, supervised visits, parenting education and intensive family reunification services.
• Participate in each client service delivery team meeting, regular supervision, client medical/behavioral health appointments as necessary and in court/county hearings.
• Help clients assess needs, acquire community services and support as necessary; listen to concerns; provide insight into positive aspects of change and problem solve crisis situations.
• Complete electronic record keeping and reporting in a timely manner as required and necessary for each client/family assigned.
• Transport or arrange transportation for clients as necessary to appointments and visits.
• Represent CONCERN at administrative/judicial reviews including termination of parental rights hearings, provide court or county ordered services to each client/family assigned and participate in family finding and group decision making efforts arranged by the county agency.
• Provide education on resources to clients and families to assist in reducing risk factors that led to referral.
• Assist with newly hired staff trainings through offering job shadowing and attend required trainings for personal development.
