Job summary

Plan A Health is seeking a Certified Nurse Assistant (CNA) to provide home care services in patients' residences. You will perform basic care, assist with daily activities, and support independence.

Qualifications

• Current certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills, able to read, write, and follow directions.
• Physical ability to lift approximately 50 lbs, bend, stand, and perform necessary tasks.
• Sympathetic attitude towards patient care and respect for patient rights.

Responsibilities

• Providing direct patient care such as personal hygiene, including bathing, shaving, hair, and mouth care.
• Assisting with transfers, repositioning, and ambulation.
• Administering basic nursing care and prescribed exercises under supervision.
• Preparing meals tailored to client preferences and dietary needs.
• Offering companionship and engaging in activities based on individual interests.
• Performing light housekeeping and maintaining accurate records of care provided.
• Monitoring and reporting changes in patients' physical or psychosocial conditions.
• Adhering to care plans, completing tasks via EVV app, and ensuring patient confidentiality.
• Supporting patient independence within their abilities and following all safety protocols.
