Job summary

Health Wealth Safe is seeking a Certified Nurse Assistant (CNA) to provide home care services across clients' residences in the service area. You will deliver direct patient care, assist with transfers, and monitor clients according to care plans.

Candidates must have a current DFS certification and a high school diploma or GED. Travel will be required; the role emphasizes safety, confidentiality, and compassionate, adaptable care without medication administration.

Qualifications

• Maintain current DFS certification.

• Monitor and report changes in clients' physical or psychosocial conditions.
• Follow the care plan precisely, including documentation via EVV app or time sheet, and uphold patient confidentiality and rights.
• Ensure safety by reporting abuse, neglect, or unsafe living conditions immediately.
