The CNA provides direct patient care in clients' homes, follows care plans, maintains accurate records, and communicates with the supervising team to ensure high-quality home health services.

Qualifications

• Must maintain DFS certification.
• HS Diploma or GED preferred.
• Ability to read, write and follow directions.

Responsibilities

• Provide direct patient care as assigned in the home setting.
• Assist with personal hygiene tasks as needed.
• Assist with transfers, repositioning and ambulation.
• Provide basic nursing care under supervision by licensed professionals.
• Prepare meals with consideration of dietary needs.
