Qualifications

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Effective communication skills to read, write and follow directions.
• Ability to perform physical tasks including lifting.

Responsibilities

• Provide direct patient care as assigned in home setting.
• Assist with personal hygiene and daily living activities.
• Guide transfers, repositioning, and ambulation as needed.
• Provide basic nursing care under supervision and report changes.
• Complete tasks according to the plan of care and EVV timekeeping.
