The home care aide provides services to patients in their residences, performing basic nursing tasks, personal care, meal preparation, companionship, and careful record-keeping while following the Plan of Care.

Qualifications

• High school diploma or GED is required or preferred.
• Maintain certification with DFS as applicable.
• Strong communication skills with the patient and family.
• Ability to perform physical tasks such as lifting, bending, and standing.

Responsibilities

• Provide direct patient care as assigned in home settings.
• Assist with personal hygiene and daily living activities.
• Help with transfers, repositioning, and ambulation.
• Provide basic nursing care and follow care plans with supervision.
