Job summary

Bluegrass Care Navigators is seeking a Part-time PACE Center Certified Nursing Assistant (CNA) to provide direct personal care to participants at the PACE Center in Lexington, KY. The role supports daily living activities, safety, and engagement in center-based activities, under RN supervision.

Requirements include a GED or high school diploma, CNA training, and KY SRNA certification, plus CPR and a food handling permit on hire. Prior elder care experience preferred, with scheduling flexibility.

Qualifications

• High school diploma or GED required; must be 18+.
• Completion of an approved CNA training program required.
• Current Kentucky SRNA certification required.
• Minimum of 1 year experience with frail/elderly population or training provided on hire.
• Food handling permit to be obtained upon hire; CPR certification maintained.

Responsibilities

• Provide direct personal care and supportive services to participants.
• Assist activities of daily living and promote safety and independence.
• Support engagement in center-based activities and documentation.
• Assist with participant transportation as needed.
