Job summary

Bluegrass Care Navigators' PACE Home Care CNA provides direct personal care in participants' homes, assisting with daily living activities, light housekeeping, and meal prep under RN supervision. This role emphasizes independence, safety, and quality of life while reporting changes to the Interdisciplinary Team (IDT).

Eligible candidates will have a SRNA license in Kentucky, completion of a CNA program, and CPR certification, with at least one year of experience with frail or elderly

Qualifications

• State SRNA certification is required.
• Completion of an approved CNA training program is required.
• Minimum of 1 year of experience with frail/elderly or training provided on hire.
• Must have a valid food handling permit (to be obtained on hire).
• CPR certification must be current.
• Must be able to work independently with minimal supervision.

As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Position Summary

The PACE Home Care Certified Nursing Assistant (CNA) provides direct personal care and supportive services to participants in their homes, supporting independence, safety, and quality of life. This role focuses on delivering one-one-one care in a home setting, including assistance with activities of daily living, light housekeeping, meal preparation, and observation of participant condition.

Working under the supervision of a Registered Nurse (RN), the Home Care CNA plays a key role in identifying and reporting changes in participants’ physical, mental, or environmental status to ensure timely intervention and continuity of care. The position requires the ability to work independently in the field while maintaining strong communication with the Interdisciplinary Team (IDT).

Position Qualifications

• High school diploma or General Education Degree (GED) required. Must be at least 18 years of age.
• Current State Registered Nurse Aide (SRNA) certification through the Kentucky Board of Nursing required.
• Completion of an approved Certified Nursing Assistant (CNA) training program required.
• Minimum of one (1) year of experience working with frail and elderly population or must receive appropriate training upon hire.
• Food handling permit required (to be obtained upon hire).
• Maintains current CPR certification.
• Ability to work independently with minimal supervision and adapt to changing participant needs and schedules.
• Demonstrated ability to provide compassionate care and effectively communicate with the adult and elderly population.
• Ability to read, understand, and follow written and verbal instructions, policies, and safety procedures.
• Ability to complete accurate and timely documentation, reports, and records.
• Ability to collaborative effectively with PACE staff and members of the Interdisciplinary Team (IDT).
• Strong interpersonal and communication skills, with the ability to interact effectively with participants, families, and interdisciplinary team members.
• Ability to solve routine problems using standardized and sound judgment.
• Ability to accommodate schedule changes based on participant needs and program demands.
• Ability to follow emergency, safety, and evacuation procedures.
• Must be a licensed driver.
