Job summary

LiveWell Homecare Agency is seeking a pharmacist to join a clinically focused team in a hospital setting. The role involves assessing, monitoring, initiating, and modifying medication therapy to optimize patient outcomes, ensuring accurate medication preparation and dispensing, and resolving medication-related issues.

The ideal candidate will have a Doctorate in Pharmacy, MO MT Services licensure, and 2–5 years of experience, with strong information resources and interdepartmental collaboration

Qualifications

• Pharmacists must hold Doctorate in Pharmacy and be licensed (MO MT services and RPh).
• 2–5 years of experience in pharmacy practice.
• Strong knowledge of drug information resources and technologies.
• Experience in inpatient and outpatient settings.

Responsibilities

• Supervising support staff and managing workflow.
• Participating in departmental meetings.
• Serving as a medication therapy resource for the team.
• Evaluating medication orders for appropriateness.
