Job summary

United Health Centers of the San Joaquin Valley is seeking a Part-Time ECM Clinical Consultant in Fresno to support non-licensed Lead Care Managers. The role oversees clinical processes within the ECM program, addressing members’ physical, behavioral, and social needs and ensuring compliant documentation.

The successful candidate provides clinical guidance, signs off on assessments and care plans, and coordinates care steps with the multidisciplinary team.

Qualifications

• Education: California RN license, LCSW, MD/DO, or NP/PA.
• Prior experience with Medi-Cal underserved populations.
• Audit/oversight experience.
• Previous Care Management/Care Coordination experience.

Responsibilities

• Coordinate billable Care Coordination activities and case conferences as directed to meet department goals.
• Ensure documentation in EHR for ECM time and billing.
• Provide training and clinical guidance to ECM staff on assessments and care plans.
• Support ECM staff in high-acuity cases and ensure audits and reviews are completed.
