Job summary

Freeman is seeking a part-time Population Health Assistant in Irvine, CA to support Coordinators in implementing programs (Anthem, Humana, UHC, Aetna, Optum) with focus on direct patient engagement, chart review and data submission.

You will collaborate with physicians and office staff to coordinate care plans, ensure program compliance, complete discharge follow-up calls and assist with the Freeman Physician Finder Line.

Qualifications

• 2 years preferred experience working in a medical setting.
• Current Certification as a Certified Medical Assistant or Registered Medical Assistant.
• Minimum 3 years clinical experience.
• AHA BLS for Healthcare Providers within 60 days.

Responsibilities

• Assist Population Healthcare Coordinators and leadership in implementing Population Healthcare Programs with focus on direct patient engagement, chart review and data submission.
• Coordinate care with physician office staff to ensure successful outcomes in utilization and care plans.
• Maintain reporting structure of all programs to ensure compliance with Freeman policies and procedures.
• Complete daily discharge follow-up calls and assist with the Freeman Physician Finder Line.
