Freeman Health System seeks a part-time Population Healthcare Assistant to support implementation of population health programs and engage with patients and physician offices. The ideal candidate holds CMA or RMA certification, has 3+ years clinical experience, and is BLS certified within 60 days.

Qualifications

• Certification as CMA or RMA is required or in progress.
• 2+ years in a medical setting preferred.
• Minimum 3 years of clinical experience.
• BLS for Healthcare Providers required within 60 days of hire.

Responsibilities

• Assist Population Healthcare Coordinators and leadership with program implementation.
• Engage directly with patients to support care plans and program goals.
• Review charts, submit data, and communicate with physician offices for care coordination.
• Maintain reporting structures to ensure compliance with Freeman policies.
• Build direct relationships with patients and office staff to facilitate outcomes.
• Perform daily discharge follow-up calls and support the Freeman Physician Finder Line.
