Job summary

Freeman Health System is seeking a Population Health Care Specialist to support the implementation of population health programs with emphasis on patient engagement, chart review, and data submission. You will coordinate with physician offices and leadership to ensure care management success.

The role requires CMA/RMA certification, at least 3 years of clinical experience, and BLS within 60 days. Part-time schedule with no holidays or weekends, 20 hours per week.

Qualifications

• Knowledge obtained through work experience that would be equal to knowledge obtained by formal education.
• Minimum 3 years clinical experience.
• AHA BLS for Healthcare Providers within 60 days.

Responsibilities

• Assist Population Healthcare Coordinators in implementing Population Healthcare Programs with focus on direct patient engagement, chart review and data submission.
• Coordinate care management by communicating with physician office staff.
• Maintain reporting structure of all programs to ensure compliance with Freeman policies.
• Complete daily discharge follow-up calls and assist with the Freeman Physician Finder Line.

Responsible for assisting in building direct relationships, communicating with patients, Primary Care Physicians and office staff to ensure successful outcomes in utilization and effective patient care plans. Responsible for completing daily discharge follow-up calls and assisting with the Freeman Physician Finder Line.**Requirements**Knowledge obtained through work experience that would be equal to knowledge obtained by formal education. 2 years preferred experience working in a medical setting. Current Certification as a Certified Medical Assistant or Registered Medical Assistant. Minimum 3 years clinical experience.
