Job summary

Qualifications

• High school diploma or equivalent required.
• Healthcare background preferred.
• One year experience in healthcare administration or member services.
• Knowledge of medical terminology.
• Proficiency with Microsoft Office and electronic documentation.
• Ability to maintain confidentiality under clinical direction.

Responsibilities

• Receive, enter, and route referrals to PHM team members.
• Contact members to schedule assessments and follow-up calls.
• Organize work queues and track tasks for clinical staff.
• Request medical records and discharge summaries from providers.
• Coordinate provider appointments, transportation, and community resources.
• Schedule interdisciplinary care meetings and distribute care plans.
• Support referrals for pharmacy and social work with information gathering.
• Prepare member/provider correspondence and educational materials.
• Maintain accurate documentation and assist with reports and audits.
• Refer urgent clinical questions to licensed staff.
