Job summary

Hope House Treatment Center is seeking a part-time, entry-level Case Manager to support patients on their recovery journey by coordinating services and guiding them to community resources. The role focuses on building discharge and aftercare plans, assisting with applications, appointments, and connecting clients with housing, employment, healthcare, and benefits.

Bilingual Spanish is a plus; a Bachelor's degree is preferred; valid driver's license required; strong organization and communication

Qualifications

• Bachelor’s degree (Master’s preferred) in social work, human services, psychology, counseling, or a related field.

Responsibilities

• Identify patient needs and barriers to recovery through psychosocial assessments.
• Develop individualized case management and discharge plans with the clinical treatment team.
• Connect patients to housing, employment, transportation, and health services.
• Coordinate referrals and assist with applications and appointments.
• Maintain compliant documentation in the electronic health record.
• Advocate for patients to reduce barriers to housing and healthcare.
• Collaborate with probation officers and community partners.
• Participate in quality improvement initiatives and outreach as needed.
• Other duties as assigned.
