Job summary

Saint Francis Ministries is seeking a Case Manager in the United States to coordinate and deliver services for our program participants. The role includes conducting assessments, maintaining regular participant contact, and developing Case Plans to support employment and education goals.

You will conduct follow-up on referrals, track milestones, and collaborate with referral sources to address barriers and achieve outcomes.

Qualifications

• Bachelor’s degree preferred in Social Work, Human Services or related field.
• Must be 21 years of age.
• Must pass organizational background and health screenings.
• Must have ability to work from home with reliable internet access.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Coordinate and deliver appropriate services to targeted program population.
• Provide weekly/biweekly participant contact to facilitate retention and access to services.
• Develop strategies to ensure follow-up to participants on referrals outside of the program.
• Chart goals, objectives, progress and milestones, including education plans.
• Provide follow-up support to assist participants with job placement and advancement.
• Provide updates to referral sources about client progress and barriers.
• Participate in monthly supervision.
• Ability to work evenings and Saturdays to meet with participants.
• Follow Saint Francis policies and SOPs and adhere to regulations and standards.
• Report unusual incidents and protect clients’ rights.

The Case Manager coordinates and delivers appropriate services to targeted program population.

• May participate in initial and ongoing assessments of clients.
• Provide weekly/biweekly participant contact to facilitate program retention, completion and access to needed support services, including services related to employment and economic stability through one-on-one meetings.
• Using assessment will determine core and supplemental curriculum to be addressed in Case Plan.
• Training and familiarity with all curriculums and willingness to facilitate/co-facilitate classes when assigned.
• Develop strategies to ensure follow-up to participants on referral services outside of the program and provide sustained support.
• Utilizes client needs and motivational interviewing to determine referrals to needed resources.
• Chart goals, objectives, progress and milestones, including employment and education plans.
• Provide follow-up support services to assist participants with job placement, retention and advancement.
• Provide updates to referral sources, sharing pertinent information about client progress, critical trends, barriers and solutions to achieving outcomes; and relationships with other organizations/communities.
• Participates in monthly supervision.
• Ability to work evenings and Saturday s to meet with participants.
• Implement Saint Francis policies and follow directives as required. Follows and adheres to all pertinent Standard Operating Procedures (SOP’s), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms.
• Is knowledgeable of and follows all safety procedures.
• Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
• Ensures clients’ rights are protected.

Requirements
