Job summary

Canyon Vista Recovery Center in Mesa, AZ is seeking a Part Time Case Manager to support treatment teams by coordinating care across providers and ensuring discharge and aftercare planning. The role emphasizes collaboration with clients, families, and community resources to implement individualized service plans.

The position requires a High School Diploma or equivalent, with a preference for a Bachelor's in Social Work or related field and experience in behavioral health case management.

Qualifications

• High School Diploma or equivalent required; Bachelor’s degree in Social Work, Counseling or related field preferred.
• One or more years of related experience in a behavioral setting preferred.
• Previous experience with the client population served by the facility preferred.
• Previous experience in case management preferred.

Responsibilities

• Develops and implements appropriate activities and/or programs within their scope of practice.
• Implements services to clients in accordance with their ISP.
• Collaborates with the client and family to implement an effective service plan, explaining options to the team.
• Maintains the client’s clinical record and progress toward recovery.
• Coordinates care with the clinical team and providers to implement the ISP.
• Ensures discharge planning begins at admission.
• Coordinates referrals for identified services via contracted providers or community resources.
• Ensures services on the service plan are evaluated and updated monthly.
• Coordinates AHCCCS/SSI/SSD applications when criteria are met.
• Ensures transition, discharge, and aftercare plans are developed prior to discontinuation of services.
• Maintains timely contact with referral sources and other treatment providers.
