Calvary Healing Center in Phoenix, AZ is seeking a full-time Case Manager to join its clinical team. You will collaborate with the healthcare staff to coordinate and facilitate quality services, focusing on treatment plans, discharge planning, and psychoeducation groups for patients in recovery.

The ideal candidate has a bachelor's degree, 2+ years in case management, and may have psychiatric inpatient experience.

Qualifications

• Bachelor's degree required; Master's degree preferred.
• Minimum 2 years of case management experience.
• 1 year of psychiatric/behavioral health inpatient experience preferred.
• Valid Fingerprint Clearance Card.
• BLS and First Aid Training completed within 30 days of employment.
• Verbal De-Escalation Training completed within 30 days of employment.

Responsibilities

• Coordinate and facilitate the delivery of quality services to patients.
• Develop and monitor treatment plans; conduct utilization reviews.
• Discharge planning and psychoeducation group facilitation.
• Collaborate with healthcare team members to support patient care.
