Job summary

Summit BHC is seeking a Part Time Case Manager to join Canyon Vista Recovery Center in Mesa, AZ. You will coordinate care across providers, support discharge and aftercare planning, and maintain ongoing communication with the treatment team and referral sources.

The role emphasizes collaboration with clients and families to implement individualized service plans and ensure services align with ISP goals. On-site position with a focus on coordinated, quality care.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s degree in Social Work, Counseling or related field preferred.

Responsibilities

• Develops and implements activities within scope of practice per individualized service plan (ISP).
• Collaborates with client and family to implement effective service plan and discuss options with the team.
• Maintains comprehensive clinical records and progress notes.
• Coordinates care with clinical team and providers to ensure ISP goals are met.
• Assists with discharge planning and aftercare planning from admission onward.
• Ensures referrals for identified services are made and coordinated within network providers.
• Handles crisis intervention as needed.
• Communicates client information to treatment team and participates in goal evaluations.
