Responsibilities

• Coordinate the interdisciplinary plan of care from admission to discharge.
• Assess patients’ conditions, needs and possible treatments.
• Advocate with affiliated agencies to secure required services.
• Design health plans including medication administration and coordination with healthcare staff.
• Collaborate with professionals to evaluate medical and rehabilitation needs.
• Identify environmental barriers to patient progress via interviews and records.
• Monitor and document patient progress against measurable goals in the care plan.
• Plan and coordinate patient care and rehabilitation using social work input and data.
• Adhere to university policies and safeguard assets.
