Texas Health Azle is seeking a part-time Care Transition Manager RN to oversee discharge planning and care coordination across Med/Surg, ICU and ER. The role requires a BSN, 3 years as a staff nurse, and preferred CCM/ACM/ANCC certifications. Cross-training to Texas Health Alliance in Fort Worth is required.

Flexible 24 hours per week with training for 4 weeks Monday–Friday 7:30 AM–4:00 PM. This position offers growth through interdisciplinary collaboration and comprehensive benefits.

Qualifications

• BSN required; ADN grandfathered for certain hires
• 3 years staff nurse in an acute care hospital
• 1 year discharge planning/care management preferred
• RN license upon hire; CPR required

Responsabilités

• Apply medical necessity criteria knowledge in discharge planning
• Collaborate with interdisciplinary teams to coordinate patient care
• Engage with patients and families to ensure smooth transitions
• Identify community resources and initiate appropriate referrals
• Maintain strong documentation of care plans and outcomes
