Job summary

The position requires a Bachelor's in Nursing or a Master's in Social Work, with 5 years of related hospital case management experience and active licensure.

Qualifications

• Bachelor's Degree in Nursing or Master's Degree in Social Work required.
• RN or LMSW/LCSW licensure required upon hire.
• Minimum 5 years of hospital case management experience with supervisory responsibilities.

Responsibilities

• Oversee Care Transition Managers (RN & SW) to ensure efficient discharge planning.
• Develop and implement policies and procedures for care transition management.
• Analyze clinical and financial data to improve LOS, avoidable days, and denials/appeals.
• Lead budget development and oversight for the care transition program.
• Collaborate with clinical partners and payors to ensure compliance and patient outcomes.
