Texas Health Resources is seeking a Director of Care Transition at Fort Worth to lead the care transition function, ensure efficient operations and outcome improvements. This role oversees Care Transition Managers (RN & SW) and partners with hospital and outpatient teams.

The position requires a Bachelor's in Nursing or a Master's in Social Work with 5 years in hospital case management and leadership experience. Full-time hours are 8-5, Monday to Friday, in Fort Worth, TX.

Qualifications

• Bachelor's Degree Nursing or MSW required.
• 5 years of hospital case management and leadership experience.
• RN, LMSW, LCSW or related licensure required on hire.
• CPR required on hire; CCM/ACM or similar preferred.

Responsibilities

• Directs daily operation of care transition to improve outcomes.
• Leads Care Transition Managers (RN & SW) and collaborates with clinical partners.
• Develops policies and staffing designs to meet system goals.
• Manages budget and uses data to drive performance improvements.
• Drives customer satisfaction and interdepartmental collaboration.
