Job summary

UTMB Health seeks a part-time RN Care Coordinator for the Clear Lake Campus to plan and coordinate safe transitions from acute care to ambulatory settings. The role requires collaboration with physicians, nurses, and support services to ensure timely utilization and quality outcomes.

The ideal candidate holds an Associate degree in nursing with 2+ years of acute care experience and a Texas nursing license; CCM/ACM‑RN certification should be obtained within 3 years of hire.

Qualifications

• RN with an Associate degree in nursing.
• Two years of clinical nursing experience in an acute hospital environment.
• Current license or valid permit to practice professional nursing in Texas.
• Certification in Care Management CCM or ACM-RN to be obtained within 3 years from the date of hire.

Responsibilities

• Plan, coordinate, and safely transition the acute care patient to the next level of care.
• Partner with physicians, nurses, and ancillary services to ensure quality outcomes.
• Coordinate discharge planning with patient, family, and medical team for a safe transition to ambulatory setting.
• Work independently and promote service excellence and accountability in care standards.
• Collaborate with disciplines, departments, payers, system partners, and community agencies to optimize outcomes.
