Job summary

Native in Bismarck, ND is seeking a part-time Care Coordinator to serve a multidisciplinary team coordinating behavioral health and human services for the Devils Lake regional area. You will facilitate weekly peer support, monthly care plans, and ensure follow-through on outcomes across health, housing and employment resources.

The role focuses on Region 7 (Burleigh, Morton and Sioux counties) with a 32-hour work week and 8am-5pm schedule, subject to change.

Qualifications

• Associate degree in human services, behavioral health or criminal justice
• At least one year of healthy living and/or recovery
• One letter of recommendation demonstrating recovery commitment
• Valid driver’s license with a good driving record
• Pass background check
• Must become state certified in Care Coordinator Training

Responsibilities

• Serve as bridge between behavioral health, criminal justice and human services
• Meet with individuals in the Community Connect program regularly
• Ensure clients obtain peer support weekly and monthly care plans
• Connect clients to medical, mental health, housing, employment and other services
