Qualifications

Responsibilities

• Direct engagement: conduct regular weekly meetings with program participants to monitor progress and provide guidance.
• Care planning: assess needs and develop participant-centered care plans.
• Inter-agency collaboration: partner with local agencies to support recovery, housing, employment, and reduce recidivism.
• Documentation: maintain timely, accurate documentation of client interactions and outcomes.
