Job summary

Better Together ND in Fargo, ND is seeking a Care Coordinator to support individuals in our recovery and housing programs. The role focuses on participant-centered care, coordinated with local agencies, and advocacy to help clients achieve physical and mental well-being.

You will conduct regular meetings, develop care plans, collaborate with care teams, and document interactions to ensure high-quality, compliant service.

Qualifications

• Bachelor's degree in a field related to the role or four years direct human services experience.
• Valid Driver’s License and ability to pass a background check.
• Commitment to complete care coordination training provided by the State of North Dakota after hiring.
• Background check clearance is required.

Responsibilities

• Conduct regular weekly meetings with program participants to monitor progress and provide guidance.
• Assess individual needs and develop a participant-centered care plan focused on client-driven goals.
• Partner with local agencies and care teams to support recovery, housing, employment and reduce recidivism.
• Maintain timely and accurate documentation of each client interaction to ensure high-quality care and compliance.
