Responsibilities include leading weekly engagement, developing care plans, collaborating with partner organizations, and meticulous documentation to maintain high-quality care and compliance.

Qualifications

• Bachelor's degree or 4 years of direct human services experience.
• Valid Driver's License.
• Ability to successfully complete a background check.
• Care coordination training to be completed after hire.

Responsibilities

• Direct Engagement: Conduct regular, weekly meetings with program participants to monitor progress.
• Care Planning: Assess needs and develop a participant-centered care plan aligned with client goals.
• Inter-Agency Collaboration: Partner with local agencies to support recovery, housing, employment, and reduce recidivism.
• Documentation: Maintain timely, accurate records of client interactions.
