Community Options ND is hiring a Care Coordinator in North Dakota to manage client caseloads in our Behavioral Health Department. You will conduct client intakes, assess needs, and develop care plans to promote client independence by overcoming barriers.

You will build strong client relationships, coordinate with service providers, and refer to internal and external resources. The role requires travel and a valid driver's license; the organization offers competitive benefits and opportunities to
