Trinity Health MI in Grand Rapids seeks a compassionate Behavioral Health Specialist embedded in primary care and specialty settings to deliver high-quality behavioral health services for patients and families, supporting mental health management and psychosocial well-being.

You will conduct assessments, provide brief therapy, crisis intervention, care coordination and consultation, collaborating with interdisciplinary teams to improve access to continuous, person-centered care, guided by NASW

Formación

• Minimum: Accredited Master’s level program in clinical social work, counseling or psychology.
• Minimum: LMSW with clinical designation or LPC or Limited Licensed Psychologist (MI).
• Preferred: Fully licensed LMSW or LPC with CAADC certification.
• Minimum: Two years of healthcare social work and/or counseling/psychotherapy.
• Preferred: Experience in ambulatory/integrated care settings.

Responsabilidades

• Conduct screenings and intake assessments, review EHRs.
• Perform biopsychosocial assessments and contribute to DSM-5-based formulations.
• Develop individualized, evidence-based care plans with clients.
• Establish measurable goals and appropriate interventions.
• Provide short-term psychotherapy, crisis intervention, and care management.
• Coordinate services with community providers and support systems.
• Document thoroughly in the EHR and support team communication.
• Supervise interns as assigned and participate in team initiatives.
