Job summary

Community Solutions is seeking a full-time case manager in Santa Maria, CA. The role focuses on intake, assessment, and referral processes to support clients on parole within a nonprofit social services setting.

You will conduct needs assessments, schedule sessions, document progress, and collaborate with partners. The position requires a Bachelor’s degree and at least one year of related experience, with strong communication and data-entry skills.

Qualifications

• Bachelor’s degree and one year experience in case management or similar field. Education may be substituted with experience on a year for year basis.
• Excellent oral, written and interpersonal skills.
• Ability to engage and develop collaborations with community agencies.
• Previous assessment experience preferred.
• Experience with Microsoft Office.

Responsibilities

• Within five days of referral, collect intake information and enter into agency data systems.
• Perform client needs assessments within five days of intake and enter data into CSI systems.
• Make referral to groups within fifteen days from intake and orientation.
• Participate in Case Planning Team Meetings and contribute to discharge planning.
• Develop and document program plan goals with clients within five days of intake and review bi‑weekly.
• Schedule, conduct, and document individual case management sessions; record in CSI systems.
• May be assigned group facilitation responsibilities.
• Enter all new referrals to interventions into CSI systems on the day of referral.
• Prepare, plan and facilitate individual and group sessions with integrity and fidelity to curriculum.
• Schedule and conduct make‑up sessions as needed.
• Document sessions in agency data systems on day received.
