Job summary

Saint John’s Program for Real Change in Sacramento is seeking a dedicated Case Manager to join our on-site team. You will assess client and family needs, develop individualized service plans, and coordinate with community resources to support goals using trauma-informed approaches.

The position requires a Social Work or Psychology bachelor’s degree with experience, or an AA and relevant experience. Strong communication, organization and teamwork are essential.

• Highly organized, empathetic, compassionate, nonjudgmental and eager to help others.

Responsibilities

• Conduct thorough assessment of client and family needs and strengths.
• Develop, implement, and review individualized service plans with clients.
• Work collaboratively with clients and with other community resources to meet/achieve client needs.
• Maintain progress notes for all clients on caseload.
• Provide case management to clients and their families, including crisis intervention, resources, and referrals.
• Facilitate psychoeducation classes and groups.
• Complete required paperwork in a timely manner.

Education

Social Work or Psychology Bachelor’s degree + 1 year experience OR AA degree + 2 years experience with individuals or families with complex needs
